Pacific Oaks office closing July 31, 2026

Our office at 220 Pacific Oaks Rd will permanently close on July 31, 2026. All providers and staff are continuing their work — they're simply moving to our Hollister Ave and Junipero St locations. Your care, your provider and your records are not going anywhere.

If you have an appointment at Pacific Oaks after July 31, it will be honored at your provider's new location. Our staff will contact you directly to confirm the details.
